Here is what I found:

1) The opening shot is on the wrong person (whose reaction is awesome, by the way)
2) The male anchor giggles
3) The female anchor gives the date instead of her name.
4) Awkward pause #1
5) Confusion as to who is supposed to introduce the field reporter
6) The female anchor introduces the wrong field reporter (she hesitantly says Jackie Win but the graphic says she's Adrienne Moore)
7) Awkward pause #2
8) As the field cameraman pans the area, the field reports tries to step into frame.

Those are just the ones I saw, there may be more. Not bad for one minute of television.

I'm going to say new producer or more likely, last minute fill-in producer? Or maybe everyone but the Anchors are new. Janitor working opening camera, new field reporter not realizing they like to pan/zoom after she starts talking, etc. At least it was the middle of the day.

rownmcinish, I missed #6. Good eye. And the reporter's reaction is priceless.
This video is the very definition of Clusterfuck.

I find that awkward pause #2 occurs every time the feed switches from the studio to a live reporter, although it did seem painfully long in this case. Everything else you found is spot on, though. And good catch on #6, I completely missed that!
